To the south west of the island starting at 5m below the surface, North Twin Pinnacle drops down to 35m. Large boulders are covered with purple soft coral and encrusting sponges. Long spiny sea urchins and a few anemones cling to the rocks as do gorgonian sea fans, sea whips and feather stars. Seahorses and ornate ghost pipefish can be found in the sea fans. Located 67 nautical miles northwest of Kawthaung and 18 kilometers north of South Twin Island, North Twin Island is a tree covered granite island which has underwater topography similar to many Similan Island dive sites. The larger and middle of the three islands is the one with the cave that has made the dive site famous. The cave starts as a large cavern entrance on the northwest corner of the island at 5m below the surface and drops down to 16m deep.
